CONTEXT Health policy has long been preoccupied with the problem that health insurance stimulates spending ("moral hazard"). However, much health spending is costly healthcare that uninsured individuals could not otherwise access. Field studies comparing those with more or less insurance cannot disaggregate moral hazard versus access. Moreover, studies of patients consuming routine low-dollar healthcare are not informative for the high-dollar healthcare that drives most of aggregate healthcare spending in the United States. METHODS We test indemnities as an alternative theory-driven counterfactual. Such conditional cash transfers would maintain an opportunity cost for patients, unlike standard insurance, but also guarantee access to the care. Since indemnities do not exist in U.S. healthcare, we fielded two blinded vignette-based survey experiments with 3,000 respondents, randomized to eight clinical vignettes and three insurance types. Our replication uses a population that is weighted to national demographics on three dimensions. FINDINGS Most or all of the spending due to insurance would occur even under an indemnity. The waste attributable to moral hazard is undetectable. CONCLUSIONS For high-cost care, policymakers should be more concerned about the foregone efficient spending for those lacking full insurance, rather than the wasteful spending that occurs with full insurance.When allocating resources, people often diversify across categories even when those categories are arbitrary, such that allocations differ when identical sets of options are partitioned differently ("partition dependence"). https://www.selleckchem.com/ The first goal of the present work (Experiment 1) was to replicate an experiment by Fox and colleagues in which graduate students exhibited partition dependence when asked how university financial aid should be allocated across arbitrarily partitioned income brackets. Our sample consisted of community members at a liberal arts college where financial aid practices have been recent topics of debate. Because stronger intrinsic preferences can reduce partition dependence, these participants might display little partition dependence with financial aid allocations. Alternatively, a demonstration of strong partition dependence in this population would emphasize the robustness of the effect. The second goal was to extend a "high transparency" modification to the present task context (Experiment 2) in which participants were shown both possible income partitions and randomly assigned themselves to one, to determine whether partition dependence in this paradigm would be reduced by revealing the study design (and the arbitrariness of income categories). Participants demonstrated clear partition dependence in both experiments. Results demonstrate the robustness of partition dependence in this context.The organization of craft production has long been a marker for broader social, economic and political changes that accompanied urbanism. The identity of producers who comprised production groups, communities, or workshops is out of reach using conventional archaeological data. There has been some success using epidermal prints on artefacts to identify the age and sex of producers. However, forensic research indicates that a combination of ridge breadth and ridge density would best identify the age and sex of individuals. To this end, we combine mean ridge breadth (MRB) and mean ridge density (MRD) to distinguish the age and sex of 112 fingerprints on Early Bronze Age (EB) III pottery from the early urban neighbourhood at Tell eṣ-Ṣâfi/Gath, Israel, dating to a 100 year time span. Our analysis accounts for the shrinkage of calcareous fabrics used to make six type of vessels, applies a modified version of the Kamp et al. regression equation to the MRB for each individual print, and infers sex by correlating MRD data to appropriate modern reference populations. When the results are combined, our analyses indicate that most fingerprints were made by adult and young males and the remainder by adult and young females. Children's prints are in evidence but only occur on handles. Multiple prints of different age and sex on the same vessels suggest they were impressed during the training of young potters. Production appears dominated by adult and young males working alone, together, and in cooperation with adult and/or young females. Vessels with prints exclusively by females of any age are rare. This male dominant cooperative labour pattern contrasts recent studies showing that adult women primarily made Neolithic figurines in Anatolia, and more females than males were making pottery prior to the rise of city-states in northern Mesopotamia.BACKGROUND Regardless of the importance of preschool vision screening (PSVS), there is limited data on the current state of these programs in Africa (particularly Ghana). This study sought to investigate the level of awareness and perception of stakeholders regarding PSVS, its availability and related policies/programmes in the Kumasi Metropolis, Ghana. METHODS This descriptive cross-sectional study included 100 systematically sampled preschools in the metropolis (using probability proportional-to-size method); 72 private schools and 28 public schools. Convenience sampling was used to recruit stakeholders of preschools (teachers, head teachers, proprietors, administrators, directors, and educationists), and were interviewed using a well-structured questionnaire. Questionnaires were administered to all eligible respondents who were present at the time of data collection. RESULTS A total of 344 respondents participated in the study; 123 (35.8%) males and 221 (64.2%) females. They form a regular network in ventricular cells, while their presence in atrial myocytes is variable regionally and among animal species We have studied the effect of variations in the TAT network using a bidomain computational model of an atrial myocyte with variable density of tubules. At each z-line the t-tubule length is obtained from an exponential distribution, with a given mean penetration length. This gives rise to a distribution of t-tubules in the cell that is characterized by the fractional area (F.A.) occupied by the t-tubules. To obtain consistent results, we average over different realizations of the same mean penetration length. To this, in some simulations we add the effect of a network of axial tubules. Then we study global properties of calcium signaling, as well as regional heterogeneities and local properties of sparks and RyR2 openings.
